Disabled Persons Concessionary Travel application form for Centro

We were recently asked by Centro, the West Midlands public transport provider, to produce a print-ready Easy Read application form for their Disability Pass.

This required several elements of the Easy Read Documents service including:

• Transcription of the original document using PhotoSymbols4 and some unique images provided by Centro

• Production of print ready document with a similar look and feel to the original plain English document

• Working with Jayne and Nikki, our in-house focus group, to ensure language and perspective was appropriate for carers and service users

• Providing a "fillable pdf" - a designed document can be completed online using the free version of Adobe Acrobat.

Although the document went through several iterations and took a bit longer than we'd originally envisaged, the final document looked very polished and professional. Centro were delighted with the results.
